ee9a396de1 Stop using -govuk-spacing(n) in SASS
Using `-govuk-spacing(n)` in the SASS files was not working, so this
replaces it with `-1 * govuk-spacing(n)` instead.

bc83ff6c09 Replace $gutter with govuk-spacing function
Replaced `$gutter` and similar variables such as `$gutter-half` with the
`govuk-spacing()` static spacing function. This uses `govuk-spacing()`
instead of `$govuk-gutter` because `$govuk-gutter` should only be used
for the gaps in between grid columns and we were mostly using `$gutter`
to add more space around elements.

There are other places in the SCSS files where we had hardcoded a
measurement in px which could be replaced with `govuk-spacing`, but this
commit only replaces the existing uses of `$gutter`.

e0ab43f988 Let GPs nominate service name
Most GP practice services are named after the practice, which is the
organisation.

So rather than make people re-type the name of their organisation (and
potentially make a typo) let’s just let them say ‘yes, that’s the name
of my organisation’.

6452676b54 Remove show/hide behaviour from permissions form
In research we found that:
- people didn’t initially realise that the permissions expanded when the
  ‘admin’ option was selected
- not having all the options visible at once makes it hard to know what
  permissions you are (and more importantly aren’t) giving to people

This commit makes it so that:
- the options within the ‘admin’ option are always visible
- a bit of Javascript logic makes it so you can pick ‘caseworker’ and
  ‘manage service’, for example (by deselecting one when you pick the
  other)

f4d2958d58 Allow setting of caseworking on a user
This commit changes the form that the user sees when inviting or editing
another user, if the service has the ‘caseworking’ permission set.

This will allow creating a new type of user, one who only has the
`send_messages` permission, without the `view_activity` permission.

We are doing this because we think there are a number of services with a
lot of users who don’t need to see the dashboard, or the other team
members, and that we can make a simpler interface for these users.
