456291d630 Put URL to service in request to go live ticket 
So that:
- we know what environment the request has come from
- a platform admin user can jump straight to that service to check it
  out

45241d6232 Put a form on the request to go live page 
This commit:
- moves things around a bit on the request to go live page
- sticks a textbox in there

So when someone click the big green button, we will get a support ticket
that looks something like:

9a2cb60f5e Update the service name validation in the ServiceNameForm and AddServiceForm to check the email_safe version 
of the name against a list of all service email_from fields.
Update find_all_service_names to find_all_service_email_from, which returns the email_from of all services.

be5aeb0676 View function for request go live was always calling update service. 
Actually this should be no op until whatever workflow will take place
in the real world is implemented.

For the moment just display flash message to say request being
processed and do nothing.

de1c0e36c8 Add /services prefix to all service-related URLs 
The URLs will be easier to parse if the parts are clearly labelled.

This is a precursor to making a blueprint for all service-related URLs.

0b62d1e755 Add confirm loop 
For pages where
- we want you to be sure that you want to do what you’re about to do
- we want to be sure it’s you trying to do the thing

This adds a page that asks the user to confirm their password.

3989d1b576 Add pages for ‘service settings’ flow 
Adds the pages and wires them together, so that it’s possible to click
through them.

The wording is not quite English, but attempts to be an rough description of
what the consequences are for each of the four actions.
