892ea01fad Update content area blocks in descendent pages
GOV.UK Frontend's template wraps all content in a
`<main>` tag and puts it in the main column.

Some of Notify's pages need to reach out of the
main column (product page).

Some have several blocks of navigation before the
main content area.

This adjusts the blocks in all the templates
where this is the case so those conditions can be
met.

6fa0f06f5d Enable autosizing for some textboxes
This commit makes textboxes auto-resize any time they might be used to
enter long passages of text which would otherwise make them scroll
internally.

8b8893ed1d Let NHS Trusts and CCGs choose own organisation
All we do via support is ask which organisation they work for and
manually assign their service to it. This commit makes that process self
service.

We think we have all the trusts and clinical commissioning groups
loaded into the database now.

This will make the go live process smoother for these teams.

e0ab43f988 Let GPs nominate service name
Most GP practice services are named after the practice, which is the
organisation.

So rather than make people re-type the name of their organisation (and
potentially make a typo) let’s just let them say ‘yes, that’s the name
of my organisation’.

daeefefeaa Let GP surgeries create their own organisations
We have a bunch of GP surgeries who want to go live. They don’t
automatically assigned to organisations. So this means a lot of back and
forth to get these organisations set up, and then the service has to
re-request to go live, and… it’s painful.

Instead, let’s let GPs create their own organisations, by confirming the
name of their organisation before going on to letting them accept the
agreement.

c9a32c7327 Remove duplication of org type lookup
There’s a couple of places where we’re looking up the label for the type
of organisation.

Having this repeated in multiple places means it’s more likely we forget
to update one of these places when making a change.

This commit looks up from the tuple in the organisation model, which is
where other code references this stuff from. This is only possible now
that we don’t have duplicate keys (ie GP practice doesn’t share a key
any more).

0916b2ba6b Require more information when creating organisations
Currently we set not-very-useful defaults for organisation type and
crown status when creating an organisation. This commit adds two field
to the form (in addition to the existing name field) to explicitly ask
for:
- organisation type
- crown status

We need these for all organisations before we can make any of their
services live.

This commit also records any new organisation as not having accepted the
data sharing and financial agreement, because if we don’t know about the
organisation already then they definitely won’t have signed it.

8b6b8c6165 Add a grid layout to the choose services page
This allows us to split the page into sections without over-using bold
fonts. And it means that when the user clicks into a service from this
page the column layout stays the same 1/4 – 3/4, rather than jumping
about so much.

It’s annoying having to go through the slow-to-load platform admin index
page to get to organisations. And it makes more sense for them to be
in the same place as other organisations and services that you can see.

da971b2da7 Allow custom notes when service goes live
This adds an option on the organisation settings page to add
'request_to_go_live_notes'. When a service belonging to this
organisation requests to go live, any go live notes for the
organisation will be added to the Zendesk ticket in the 'Agreement
signed' section.

d9da63401f Normalise heading sizes
Since we added template folders the templates page has had a ‘medium’
sized heading, where other pages have stuck with a ‘large’ size.

This commit rationalises the decision around which pages have which
heading size:
- ‘navigation’ pages (eg templates, team members, email reply to
  addresses) have medium sized headings
- transactional pages (ie ones which have a green button) keep the
  larger heading size

74fb30ce5f Add GOV.UK Design System style back links
The Design System has standardised on back links being at the top of the
page, decorated with a small text-coloured arrow.

I think this makes more sense than having them at the bottom, because it
suggests, in some way, being able to go back before commiting to any of
the forms on the page. Whereas the things at the bottom of the page
should be performing actions on what’s in the page.

The reason for making this change now is that it de-clutters the area
around the green buttons. This was presenting a design challenge where
multiple levels of interaction were happening in the same form. Moving
these back links to the top of the page should mean that, in these
complicated forms, there’s one fewer thing to compete for the user’s
attention.

I’ve componentised this into a `page_header` macro so that the change is
easier to roll out and maintain.

da19c0b4ad Remove bullets from lists in tables
I think this is something we inherited from the Digital Marketplace
code. We only use this for organisation settings are the moment, but
the list markers are redundant because each item will never wrap onto a
new line; it will truncate instead. Still keeps a little sliver of
spacing just so it doesn’t look like a paragraph.

5ac2abb7bc Make center column of table wider
Most of the content of our ‘settings’ tables is in the value, not the
key. The value is in the middle column. So we should allocate the most
space to the value.

The previous layout was based on the premise that most pages divided the
grid like this:

This was because a lot of pages had a left column for emails, and a
right column for text messages, so it felt consistent for tables to
always default to 50% of the width of the main column.

This consistency has faded with time, especially as we added letters.

So this commit changes these tables to allocate more space to the
central column, but still sticking to the grid like this:

Since there’s more space to display the value of a setting this commit
also truncates settings that are too long to fit in the width of the
column (for example a long email address) rather than the previous
behaviour of truncating them. This all just makes things look a bit
cleaner.

e1197c54a5 Don’t allow indexing on service model
Making people use a property is a sure way to make sure they’re spelling
the name of the property correctly, and allows us to easily swap out
properties that call through to the underlying JSON, and properties
which are implemented as methods.

d9da219b7e Use a macro for form tags
This will stop us repeatedly forgetting to add `novalidate` and
`autocomplete='off'` to our forms (which is how most of them are set
up).

It uses sensible defaults, based on how we most-commonly configure
forms:
- most of our forms are `post`ed (but this can be overridden)
- `autocomplete` should only be enabled where it makes sense, otherwise
  it’s more annoying than useful (but this can be overriden)
- we should never be using HTML5 form validation because our own error
  styles and messages are better
