581296b95e Fix off-by-one dates on provider history
We chunk history entries by their YYYY-MM-DD date representation to
display them day-by-day.

However trying to convert a YYYY-MM-DD string into a timestamp is
unpredictable, and was causing our dates to be one day out (probably
because of midnight/daylight savings/general datetime horrors).

This commit changes the code to do the same thing as the history page,
which is look at the `updated_at` field on the first entry to get a
datetime object and from that the formatted date we show in the headings
on the page.

8122fa1d5d Simplify and add tests
Makes these assumptions:
- we don’t care about history before 29th November 2019 at 11am (this
is when priority started to mean a proportion rather than a ranking)
- the priority of the second provider will always be the inverse of
  the first provider

Which means the code is a lot simpler/actually does what you’d expect.

5233ee4bd9 Add a form to set priority of top 2 providers
Their priority should always add up to 100%. Currently we have to ensure
this by hand. Adding this form means there’s no way to not set their
combined priorities to 100%. And it’s a bit more of an intuitive UI than
two textboxes on separate pages.

16ebdfeb8b Add page showing priority of two SMS providers
It’s not very useful to know the priority of one provider without
knowing the other. And these pages were never really designed, so they
weren’t super easy to understand anyway.

This commit adds a page that takes the first two text message providers
and shows their relative priority against each other.

It follows the design of the events page, as a pattern for showing a
log of historical events.

259a82e066 Update for new format of data returned from api
- API will now send through `created_by_name` instead of `created_by`.
- API will always send through `current_month_billable_sms` but this can
now be `0` instead of `None`.

74fb30ce5f Add GOV.UK Design System style back links
The Design System has standardised on back links being at the top of the
page, decorated with a small text-coloured arrow.

I think this makes more sense than having them at the bottom, because it
suggests, in some way, being able to go back before commiting to any of
the forms on the page. Whereas the things at the bottom of the page
should be performing actions on what’s in the page.

The reason for making this change now is that it de-clutters the area
around the green buttons. This was presenting a design challenge where
multiple levels of interaction were happening in the same form. Moving
these back links to the top of the page should mean that, in these
complicated forms, there’s one fewer thing to compete for the user’s
attention.

I’ve componentised this into a `page_header` macro so that the change is
easier to roll out and maintain.

d9da219b7e Use a macro for form tags
This will stop us repeatedly forgetting to add `novalidate` and
`autocomplete='off'` to our forms (which is how most of them are set
up).

It uses sensible defaults, based on how we most-commonly configure
forms:
- most of our forms are `post`ed (but this can be overridden)
- `autocomplete` should only be enabled where it makes sense, otherwise
it’s more annoying than useful (but this can be overriden)
- we should never be using HTML5 form validation because our own error
  styles and messages are better

cfc572d4f7 Add some internal navigation for platform admin
There are way too many links in the top nav now (when you’re using the
site as a platform admin).

And how often are you going to look at the letter jobs page really?

This commit adds some internal navigation for the platform admin area of
the platform, copying the style that works for navigating a single
service.
