dc6d66daba Make people sign in to get the agreement
Currently we have a bunch of users who aren’t signed in asking us for
the agreement.

This is bad because:
- it’s slower (for them) than just being able to download it
- it creates work for us

We can’t just offer the agreement to anyone, but we can offer to it to
anyone who’s signed in because we now let people self-select which
version to download when we can’t tell which one to give them.

We prefer people downloading the agreement if they can. If we don’t know
which agreement they should be using (ie we don’t know their crown
status) then we fall back to having them contact us.

64b5f03dcd Don’t populate support form with arbitrary text
I don’t think it’s a massive risk (we’re certainly mitigating against
any XSS), but having a page on a GOV.UK domain where you can prefill
text on the page from a query string probably isn’t great.

So this commit restricts prefilling the support form to a set of
named questions.

6c47375d9f Make terms page smarter about the agreement
People are emailing us asking if their organisation has signed the
agreement. In some cases they have, so this is a waste of their and
our time.

This commit adds a bit of logic to the terms of use page to tell users
when their organisation has already signed the agreement.

7bbd16e78f Simplified to links to guidance in the service manual
Some strange pages to be linking to. Most of the things we are trying to say are now covered on this page https://www.gov.uk/service-manual/design/sending-emails-and-text-messages, so much tidier and more effective to link just to that.

135ff1ee77 Include service name in page <title>
In pages specific to a service (e.g. dashboard and sub pages) the title
needs to distinguish which service it applies to. This is mainly to give
context to screen reader users who could be managing multiple services.

Implementing this uses template inheritance:

`page_title` includes `per_page_title` includes `service_page_title`

‘GOV.UK Notify’ is inserted into every page title.

Pages that set `service_page_title` get the service name inserted too.

1df3c11ae9 Split support into two pages
The kind of communications we’re getting at the moment can broadly be
broken down into:
- problems
- questions and feedback

We will need to triage problems differently, because they could
potentially be urgent/severe/emergency/P1/whatever language we use.
Questions or feedback will never be P1.

Two reasons for making the user categorise their tickets themselves:

- Outside of hours we can’t get someone out of bed in order to decide if
  a ticket is a problem or just feedback

- We can tailor the subsequent pages to whether it’s a problem or
  feedback (eg showing a link to the status page if the user is having
  a problem)

This commit let’s users make the choice with a pair of radio buttons.

It also cleans up a bunch of the tests and parameterizes them so we’re
testing the flow for both ticket types.
