79053dec93 Allow uploaded letters to be sent if valid
Added a send button which only appears on the page if the query string
indicates that the PDF is valid. Before actually sending, we check that
the service has the right permissions and that the metadata for the
letter confirms the letter is valid (because the query string can be
changed).

7368245c9a Show letter preview once file is uploaded
This shows the sanitised letter preview if the file had no validation
errors or the preview with the overlay if it failed validation.

c577416866 Add letter upload form which redirects to blank preview page
Added a form to upload a single letter. Currently this only uses the
form to validate that a file is submitted and that the file is a PDF. If
either of these validations fail, the form will display an error.
Otherwise, we redirect to a new preview page which just has the filename
as the heading for now.

8b8893ed1d Let NHS Trusts and CCGs choose own organisation
All we do via support is ask which organisation they work for and
manually assign their service to it. This commit makes that process self
service.

We think we have all the trusts and clinical commissioning groups
loaded into the database now.

This will make the go live process smoother for these teams.

daeefefeaa Let GP surgeries create their own organisations
We have a bunch of GP surgeries who want to go live. They don’t
automatically assigned to organisations. So this means a lot of back and
forth to get these organisations set up, and then the service has to
re-request to go live, and… it’s painful.

Instead, let’s let GPs create their own organisations, by confirming the
name of their organisation before going on to letting them accept the
agreement.

8a453890e6 Add 'Uploads' hub and navigation
The uploads hub is just a page with text for now - there are no actions
available on the page. It is linked to from a new 'Uploads' menu item on
the left of the page which is only visible if your service has the
`letter` and `upload_letters` permissions and if the current user has
permissions to send messages.

9dc13f1d8e Add new report to show monthly notification stats for each service
This report will be used by the engagement team. There is a form to give
a start and end date for the report, and the form is then downloaded
as a CSV file when the form is submitted.

44d5dc44d3 Allow deleting default letter contact blocks
It’s possible to delete default letter contact blocks because there is a
fallback – having a blank letter contact block. This is different to SMS
senders and reply to addresses.

For this to make sense it also means:
- adding the ‘blank’ letter contact block to the list of letter contact
  blocks
- having a way of setting the default back to being blank

53214937a8 Stop allowing the service org type to be changed
The service organisation type will either be the same as the org type of
the service's organisation or will be set by a user when creating a new
service. This removes the ability to change it from the platform admin
settings table.

a256b9c33a Remove the user-specific agreement pages
We used to give users the right version of the agreement by guessing
their organisation from their email address.

Now we do it by looking at the organisation of the service they’re
looking at.

In other words, users should only be downloading the agreement as part
of the go live journey, not outside it. This is because we think that
users will get confused if they download the agreement and:
- find there’s nowhere to physically sign it
- think that accepting the agreement is all they need to do to go live

Maintaining two paths to download the agreement also makes the code more
complicated, and makes it harder to update the content on these pages.

A user might not have a guessable organisation type, even if the service
they’re working on does have an organisation set. This can happen for
users with @nhs.net email addresses, for example.

eb3f9aad2a Add pages to let users accept the agreement online
At the moment, the process for accepting the data sharing and financial
agreement is:

1. download a pdf
* print it out
* get someone to sign it
* scan it
* email it back to us
* we rename the file and save it in Google Drive
* we then update the organisation to say the MOU is signed
* sometimes we also:
 * print it out and get it counter-signed
 * scan it again
 * email it back to the service

Let's not do that any more.

When the first service for an organisation that doesn't have the
agreement in place is in the process of going live, then they should
be able to accept the agreement online as part of the go live flow. This
commit adds the pages that let someone do that.

Where the checklist shows the agreement as **[not completed]** then
they can follow a link where they can download it (as happens now).
From here, they should then also be able to provide some info to accept
it. The info that we need is:

**Version** – because we version the agreements occasionally, we need to
know which version they are accepting.  It may not be the latest one if
they downloaded it a while ago and it took time to be signed off

**Who is accepting the agreement** – this will often be someone in the
finance team, and not necessarily a team member, so we should let the
person either accept as themselves, or on behalf of someone else. If
it's on behalf of someone else we need to the name and email address of
that person so we have that on record. Obvs if it's them accepting it
themselves, we have that already (so we just store their user ID and
not their name or email address).

We then replay the collected info back in a sort of legally
binding kind of way pulling in the organisation name too. The wording
we’re using is inspired by what GOV.UK Pay have. Then there’s a big
green button they can click to accept the agreement, which stores their
user ID and and timestamp.

3511f8ad5b Move organisations out of platform admin area
It’s annoying having to go through the slow-to-load platform admin index
page to get to organisations. And it makes more sense for them to be
in the same place as other organisations and services that you can see.

00bb7a0ea0 Add page to archive user
Users can only be archived by Platform Admin from the user page
(/users/<user_id>). This removes them from all services and orgs and
updates their details.

da971b2da7 Allow custom notes when service goes live
This adds an option on the organisation settings page to add
'request_to_go_live_notes'. When a service belonging to this
organisation requests to go live, any go live notes for the
organisation will be added to the Zendesk ticket in the 'Agreement
signed' section.

35bf42b024 Add service-specific versions of agreement page
Our usability testing found that jumping out of the service when going
to download the agreement made it difficult for people to find their way
back to the ‘Request to go live’ page.

This commit adds a duplicate, service-specific versions of these pages
which have the same content but:
- keep the service navigation
- have a link back to the ‘Request to go live’ page

f872294235 remove the remove-user get method
remove `confirm` from `confirm_remove_user_from_service` as there's
only one action now that the initial confirmation prompt takes place

f7f9dd8530 fix user permissions save button sometimes deleting
when you hit the delete button, it flashes the delete button and takes
you to the `/service/../user/../delete` url. If you then click the save
button, it would make a POST to the delete URL... and delete the user.

now the page stays on the edit url, but adds a `?delete=yes` query
string. The dangerous flash banner now has an action field which
defines where the browser will make the POST to (which remains at
/delete).

8fd5fa90ff Make the letter-validation-preview url public.
In the short term I have created a duplicate version of the letter-validation-preview so that people from a service can upload a pdf and see why the letter isnot validating.

It's hard to get a precompiled letter to validate when starting to integrate with Notify. This will return the overlay of the letter validation and is now available to the services.
At the moment they send us a PDF to upload.
This is temporary because there is a story to create a one-off flow to get this overlay, that will replace this page.

There is no navigation to this on purpose.

8791134c60 Move the ‘estimated usage’ questions
We get a bunch of requests to go live where people have told us they're
going to send email but there is no email reply-to address present.

These come from 2 scenarios:

1. when there are email templates, and no reply to address – but they
   ignore the checklist
2. when there are no email templates (yet) but they provide anticipated
   volumes for email

At the moment we only auto-check for a reply to address when they have
email templates. And because the question about anticipated volumes
follows the checklist, you'll get a checklist that passes (reply
addresses not required as no templates present) - but your future intent
that differs (reply address IS required because you have anticipated
volumes).

So let’s bring the request for anticipated volumes into the checklist,
that way we can dynamically add the requirement for a reply to address
if they say they will send email but don't have templates yet.

We should begin storing it in the database against the service to stop
people having to re-enter it each time they try to complete the go live
screens.

This also means moving the ‘consent to research question’ along with
the questions about volume, because
- we want people to answer both before going live
- we don’t want to clutter up the summary page by asking questions there
  too

000d6c3a48 Replace platform admin service setting buttons with forms
Most of the existing platform admin buttons on the service settings
page used to issue GET requests to switch service settings. This
means they weren't protected by CSRF. On top of that as our number
of service permissions increases over time a lot of buttons on the
page made it hard to work with.

To fix these issues we replace most of the buttons with rows in the
platform admin settings table. Each setting has a 'Change' link that
leads to a page with an On/Off switch form.

This removes "research mode" switch completely since we're planning
to deprecate it in the future and we don't expect to switch any new
services into research mode at the moment.

Most service permissions are now handled by a shared endpoint that
is parameterized with the permission name. Some permissions that
require some additional logic before they can be toggled (like document
upload, which requires setting a contact address) have separate
initial endpoints that redirect to `set_service_permission`.

"Archive", "Suspend" and "Resume" actions are kept as buttons since
they display a confirmation banner (which is a CSRF-protected form)
and they're not easily represented as an On/Off switch.

d904c7e5c5 Show letter branding preview when selecting a brand
This adds a preview pane which is visible when updating a letter brand.

If JavaScript is enabled, the preview pane shows on the set-letter-branding
page, and submitting the form saves updates the letter brand for a service
immediately. If Javascript is not enabled, there is a separate 'Preview email
branding' page which shows a preview of the brand and has a 'Save' button on it.
