adcd798d88 Catch error when organisation domain already in use
Up till now, when adding new organisation domain, if it was already
in use, we didn't handle the 400 we got back from API. This PR
adds handling for that error.

8215022725 Show 2 previous financial years on usage page
From a question on cross-government Slack:

This seems like a reasonable idea, and is something we’ve talked about
before. The original intention[1] was that seeing your (unchanged) free
allowance for next year would be useful, but that doesn’t really seem to
be a user need.

ae8dd8e8cd Tidy up the platform admin pages
This commit does two things:
- brings the ‘All organisations’ page back within the platform admin
  part of the site (because it’s hard to find otherwise)
- makes the layouts of all the pages within platform admin a bit closer
  to the service-specific pages in terms of heading sizes, spacing, etc
  so that moving between them doesn’t feel so jumpy

65bb72ef2f Bump WTForms to 2.3.1
This involves three changes which broke our code.

To validate email addresses, the optional dependency `email-validator`
must be installed<sup>1</sup>. But since we don’t use WTForms’ email
validation, we shouldn’t need to subclass it – it can just be its own
self contained thing. Then we don’t need to add the extra dependency.

When rendering textareas, and extra `\r\n` is inserted at the beginning
<sup>2</sup>. Browsers will strip this when displaying the textbox and
submitting the form, but some of our tests need updating to account for
this.

The error message for when you don’t choose an option from some radio
buttons has now changed. Rather than just accepting WTForms’ new
message, this commit makes the error messages like the examples from
the Design System<sup>3</sup>. By default it will say ‘Select an
option’, but by passing in an extra parameter (`thing`) it can be
customised to be more specific, for example ‘Select a type of
organisation’.

cc5701e870 Cache organisation name in Redis
A lot of pages in the admin app are now generated entirely from Redis,
without touching the API.

The one remaining API call that a lot of pages make, when the user is
platform admin or a member of an organisation, is to get the name of
the current service’s organisation.

This commit adds some code to start caching that as well, which should
speed up page load times for when we’re clicking around the admin app
(it’s typically 100ms just to get the organisation, and more than that
when the API is under load).

This means changing the service model to get the organisation from the
API by ID, not by service ID. Otherwise it would be very hard to clear
the cache if the name of the organisation ever changed.

We can’t cache the whole organisation because it has a
`count_of_live_services` field which can change at any time, without an
update being made.

d2d5510376 Add a search as you type form
As in other places where we have a long list of things where you might
want to jump to a specific thing. We use this pattern where there are
more than 7 things, per the magical number 7[1]

bd9e127e57 Sum up usage for an whole organisation
We invoice on a per organisation basis, so it’s useful to know the per
organisation figures without needing to do any spreadsheet-fu.

4d89d36847 Use big numbers for organisation usage
This is the pattern we use to display counts of things on the dashboard
and usage pages. It does some nice stuff like dealing with
comma-separating and formatting monetary amounts.

This commit also adds some logic to show the free allowance used if the
service hasn’t spent anything on text messages yet.

f3bb93f460 Replace straightforward buttons and links styled like buttons
This replaces the buttons that aren't part of a macro and that we don't
need to write additional styles for with their govuk-frontend equivalent.

There were some links that were styled to look like buttons, so these
have also been replaced with the new govuk-frontend macro.

There was one button on the `choose-account.html` template that was in a
section of code that was never reached - this has been deleted.

it. So this commit renames the property to better describe what it is

e4134072d9 Reduce the errors related to user fixtures
We were using user fixtures in a lot of parameterized tests, but this is
no longer allowed in Pytest 5. To avoid having to split up the parametrized
tests (which would make the test files a lot longer and slightly more
difficult to read) this commit creates functions which return various types
of user json so that we can use these as the test parameters instead.

8b8893ed1d Let NHS Trusts and CCGs choose own organisation
All we do via support is ask which organisation they work for and
manually assign their service to it. This commit makes that process self
service.

We think we have all the trusts and clinical commissioning groups
loaded into the database now.

This will make the go live process smoother for these teams.

Most GP practice services are named after the practice, which is the
organisation.

So rather than make people re-type the name of their organisation (and
potentially make a typo) let’s just let them say ‘yes, that’s the name
of my organisation’.

daeefefeaa Let GP surgeries create their own organisations
We have a bunch of GP surgeries who want to go live. They don’t
automatically assigned to organisations. So this means a lot of back and
forth to get these organisations set up, and then the service has to
re-request to go live, and… it’s painful.

Instead, let’s let GPs create their own organisations, by confirming the
name of their organisation before going on to letting them accept the
agreement.

38c2b32fa8 Add ‘GP’ as an organisation type
Although their allowances are the same as what we call `nhs_local` it
makes more sense to store them separately because:

- we already present them as two separate choices to the user
- we may want to handle them differently in the future, eg in terms of
  what branding choices are available to them

Once the API is updated we can start passing in this new value from
the admin app.

0916b2ba6b Require more information when creating organisations
Currently we set not-very-useful defaults for organisation type and
crown status when creating an organisation. This commit adds two field
to the form (in addition to the existing name field) to explicitly ask
for:
- organisation type
- crown status

We need these for all organisations before we can make any of their
services live.

This commit also records any new organisation as not having accepted the
data sharing and financial agreement, because if we don’t know about the
organisation already then they definitely won’t have signed it.

8b6b8c6165 Add a grid layout to the choose services page
This allows us to split the page into sections without over-using bold
fonts. And it means that when the user clicks into a service from this
page the column layout stays the same 1/4 – 3/4, rather than jumping
about so much.

628e344b36 Make user API client return JSON, not a model
The data flow of other bits of our application looks like this:

The user API client was architected weirdly, in that it returned a model
directly, like this:

    API client (returns a model, of type `User`, `InvitedUser`, etc)

                         View (returns HTML)
```

This mixing of different layers of the application is bad because it
makes it hard to write model code that doesn’t have circular
dependencies. As our application gets more complicated we will be
relying more on models to manage this complexity, so we should make it
easy, not hard to write them.

It also means that most of our mocking was of the User model, not just
the underlying JSON. So it would have been easy to introduce subtle bugs
to the user model, because it wasn’t being comprehensively tested. A lot
of the changed lines of code in this commit mean changing the tests to
mock only the JSON, which means that the model layer gets implicitly
tested.

For those reasons this commit changes the user API client to return
JSON, not an instance of `User` or other models.
