efe7e2f35e Add JS modules support & use for GOVUKFrontend
The JS for GOVUKFrontend components is available
individually so you can only include what you
need:

This uses the JS Modules syntax:

Our JS is delivered as one file so we need to use
a bundler to convert the modules to a single file.

This adds a build step to transpile all modules
into a single file, which is then added to the
files combined into the one that get delivered.

Rollup is used as the simplest bundler to use for
this purpose. It also introduces the least
boilerplate JS.

Note: the CommonJS plugin is needed as GOV.UK
Frontend components are published as UMD modules.

In future, this work should let us work on this
story dependencies:

278a70ecc7 Remove GOVUK Template files
Includes Sass that targeted GOV.UK Template HTML
and also moves some link styles to `globals.scss`.

Also removes bits of frontend build that copied
over GOVUK Template files.

7f0b207f0a Put cookies message back in
The cookie_message block was part of GOV.UK
template but is not included in the GOV.UK
Frontend template.

This adds it back in along with JS to set the
cookies from GOV.UK template and styles, taken
from the Design System's website (which I assume
has the right colour contrast).

from GOV.UK Elements (which uses JS from the
GOV.UK Frontend Toolkit).

905dce2f49 Replace removed global GOV.UK Template styles
GOV.UK Template included styles targeting elements
by their tag name, so affected all elements of
that type. Removing these files removes certain
styles from the top of the cascade.

Moving all components to GOV.UK Frontend (that are
possible) should fix this further down the
cascade. This adds them back to plug the gap in
the cascade until that work happens.

6957a95121 Set vertical spacing for <main>
GOV.UK Frontend gives `<main>` different padding
on the top and bottom to us.

This adds our spacing to `<main>` for pages
without a side nav (except the product page which
is special) and to the row containing the `<main>`
and side nav sections on pages that have a side
nav.

e366bdadb0 Make <main> section position: relative
The content of the GOV.UK Frontend header
component is `position: relative`. This changed
the [default z-index
stacking](https://developer.mozilla.org/en-US/docs/Web/CSS/CSS_Positioning/Understanding_z_index/Stacking_without_z-index_
so it was above `<main>`.

This aligns `<main>` with the header, meaning
`<main>` appears above it due to being below it in
the document.

d80b42a255 Add GOV.UK Frontend JS
This includes the JS for all GOV.UK Frontend code.
If our frontend build includes a module bundler in
future, we should only include the JS for the
components we use, as with our Sass.

1c3095329b Change radio buttons to inline
Adds ability to have inline radio buttons using the fieldset.inline
functionality from gov.uk elements.

Then implements this for the radio buttons for choosing postage
class.

Also overrides the gov uk elements styling for the inline radio
buttons to place them slightly closer together as this looks
better.

HighlightTags was bad because:
- we haven’t called placeholders ‘tags’ for a long time
- it also does resizing of the `<textarea>`, not just highlighting the
  placeholders

d0ce4d07a3 Autosize textboxes w/out highlighting placeholders
Scrolling within textareas on the page is a bit grim. Which is why we
don’t do it for the textboxes that people use to edit templates.

This commit will allow us to extend the auto-resizing of `<textarea>`s
to those which don’t need the highlighting of placeholders.

The code is still quite coupled to the placeholder highlighting code,
because both work by copying the content of the `<textarea>` into a
`<div>` that underlaps the textbox. This `<div>` is used for both
rendering the placeholder highlights, and calculating the natural height
of the content. So it would be hard/confusing to split the two bits of
code into separate modules.

4e026bc306 Make sure all sticky footer variants are aligned
The bottom margin should always be consistent. Height of footers doesn’t
have to be consistent. Items on the left and right should baseline
align.

2732a0b80b Make things line up on the usage page
The mixture of three column/two column layouts on this page has always
looked a bit disjointed. And since the left column will only even
contain the names of months, which are short, it doesn’t need a full
half of the page width.

8a5e7a00dc Remove min-height from sticky submit buttons
It makes them get out of alignment with other things in sticky footers.

The problem of buttons getting too close to the bottom of the window can
be fixed with some min-height on the container instead.

b2ebaf153a Chunk events by day
Scanning the page is difficult at the moment because it’s hard to tell
how far apart in time events are, and thereby determine which events
might be related.

Grouping the events by day quickly lets users narrow their focus to
a meaningful subset of the events.

5d82dc8b36 Make text transparent on conditional placeholders
Otherwise you have a visible copy of the text underlapping the text in the textbox. Which, when they don’t quite align makes the text look bold. Seems to be more noticeable on some browsers/operating systems than others, but a bug all the same.

d0b6f844ce Move focus of legends to their fieldsets
When testing with the JAWS screenreader, we found
a bug around getting it to announce the name of a
fieldset when we ask the user to select from it.

Bug on pivotal:

The flows for adding a new template and moving a
template/folder both need the user to select an
option from a radio group. When we add the radio
group to the UI, we need to move focus to it so
the user is in the right place to choose an
option.

The expectation of the original code was that
focusing the field set's legend would work like
focusing the heading of a section of content and
announce the label of it. This didn't happen with
JAWS. This tries to achieve the same by focusing
the whole fieldset instead.

When doing this we also hide the focus style, to
follow the convention for this across www.gov.uk.

e0ab43f988 Let GPs nominate service name
Most GP practice services are named after the practice, which is the
organisation.

So rather than make people re-type the name of their organisation (and
potentially make a typo) let’s just let them say ‘yes, that’s the name
of my organisation’.

cc713b4057 Remove code specific to IE8 and below
Our usage for these browsers in the last month is down to 0.2% of all
users, or 14 individual users, according to Google Analytics.

These users also visit about half the number of pages per sessions,
suggesting that they’re not signed in.

92626c3f41 Change method of hiding extra table layer
This module creates a clone of the existing table
as an extra layer which sits on top of it.

This allows the row headers to sit above the table
content when it scrolls.

We were setting `role=presentation` on the extra
table to hide it from assistive technologies.

It seems that this wasn't working. From the spec'
`role-presentation` should remove the semantics of
the table but leave the content.

Testing in Safari, with the OSX Accessibility
Inspector, this isn't happening and the table is
still being reported as a table by the
accessibility API.

This changes to the `aria-hidden` attribute, to
make sure the extra table is ignored.

c11c054323 Fix option selection for keyboard users
Keyboard users select a time slot by moving to the
radio for that slot, using the arrow keys, and
selecting it by pressing 'space' or 'enter', like
a `<select>`.

We allow this by listening for 'keydown' events
from the 'enter' or 'space' keys on time slot
radios that are checked.

Browsers fire 'click' events alongside the
'keydown' event meaning it's possible for the
code that makes the selection to be run twice.

We currently guard against this by checking for
the `pageX` property of the event object,
reasoning that a click event fired by a key press
won't have a cursor position.

Most browsers we support set it to `0` but it
isn't always the case:

For those browsers, the `!event.pageX` condition
resolves correctly so this works. Safari and
versions of Internet Explorer before 11 however,
set it to a positive number.

In those browsers, moving the selection between
radios using the arrow keys fired a 'click' event
which, in Safari and IE<11, was treated as a
mouse/touch event and so confirmed the selection.
This made it impossible to select a later time.

These changes replace the 'click' event on time
slots with an artifical one that tracks
mouse/trackpad clicks by listening for a
'mousedown' followed by a 'mouseup' on a time
slot. This doesn't fire on key presses so avoids
the problem.

64c6d1fbc7 Make clicking 'Done' preserve any selection made
Clicking the 'Done' button resets the module to
its default state. 'Done' implies you've
completed your selection so this doesn't make
sense.

This changes it so any selection made will be
confirmed when 'Done' is clicked.

It helps the tests to know the `Hogan` variable is
actually a property of the global variable
(`window` in this case) and doesn't hurt the
readability of the script.

7901d08781 Style soft folder separators
If you can see a folder but not its parents we concatenate the
breadcrumb into one link.

This styles folder separators inside these links a bit differently to
make them do a bit less visual separation than the ones outside the
links.
