cdb5b47c4d Post to the API when moving folders and templates
This commit adds logic to:
- take the list of selected folders and templates
- split it into two lists (of folders and templates)
- `POST` that data to the API, to effect the movement of said folders
  and templates

I’ve tried to architect it in such a way that we can easily add more
template ‘operations’ in the future, as we add more forms to the choose
template page.

2743216519 Show form elements for templates and folders
This commit adds:
- checkboxes to let you select a template or folder
- radio buttons to let you select where to move those template(s) and/or
  folder(s) to

It only does the `get` part of this work; handling the `post` and
calling API will be done in a subsequent commit.

9aa4a7267a Use <button> not <input>s for form submission
Both `<button type='submit'>Submit<button>` and
`<input type='submit' value='Submit'>` can be used to submit a form.

We have historically[1] used `<input>` because it’s better-supported by
IE6 in that:
- the `submit` attribute is mandatory on `<button>`, not on `<input>`
- the `innerHTML` of a button will be submitted to the server, not the
  value (as in other browsers)

Reasons to now use `<button>` instead:
- IE6/7 support is no longer a concern (especially with deprecation of
  TLS 1.0 on the way)
- Because an `<input>` element can’t have children, the pseudo-element
  hack[2] used to ensure the top edge of the button is clickable doesn’t
  work. We’re seeing this bug[3] affect real users in research.

d01c397bb4 Allow creation of an organisation without a logo
Now we have the org banner branding, not all organisations need a logo.
So it shouldn’t be an error to not provide one.

ac948ceb19 Make delete link flush left when there’s no button
The delete link was designed to be used with a button, where it needs
some padding to separate it from the button.

We now have a case where it’s being used without an accompanying button,
so we need a variation without that padding.

97a3bf9225 Remove the ‘manage templates’ page
The ‘manage templates’ page was almost identical to the ‘send text messages’
page.

This commit consolidates them into one and makes them all hang together.

Part of this means tweaks to the javascript so that files upload as soon as
you’ve chosen them.

f5807d939a Add secondary link pattern to page footer
This commit brings back the ‘link under the green button’ bit of the page footer
component.

Previous it had been changed to be a grey button. But there are use cases for
both, maybe even simultaneously.

This is a link not a button because:
- it’s less prominent—delete is an infrequent action
- it’s a two-step process, and only the second part changes any data (so it has
  a button)

4d10eec1e2 Rename submit_form component to page_footer
Submit form was
- a confusing name in itself
- not descriptive, because it also has an optional ‘back’ link

This commit also puts this component in as many pages as possible, stripping
out any hard-coded buttons and links.

It replaces any usage of buttons for ‘back’, because these are links, not
buttons (ie they don’t change any data).
